Submitting Forms

In an effort to protect residents, Springville City Power department requires the Commercial Customer-Owned Generation Agreement form to be signed and submitted to the Power department, in person by the account holder.

The Application for Commercial Customer-Owned Generating System License must be filled out and submitted in person, by the account holder, to the Power department.

A building permit must be obtained for the installation of systems or other structural changes from the Building and Inspections department. For information on submitting a building permit, please click here.
