All non-residential users of the Springville wastewater treatment system are required to submit wastewater questionnaire. Information given in the questionnaire will be used to establish if a Wastewater Discharge Permit is required.

If there are any changes in the information contained in the original submitted questionnaire or changes in the numerical values outside of the ranges stated in the questionnaire, the user is required to submit the changes, in writing, within 30 days of occurrence.
